Students receiving a degree/diploma/certificate (credential) must fill out a Credential Application. These forms are available on the website and in the Registrar’s Office. After filling out the form, students must meet with their advisor to make sure all program requirements have been completed. The advisor then signs the Credential Application, attaches all necessary documentation, and forwards the application packet the Registrar's Office. The Registrar then does a credential audit to ensure that all program requirements have been met and all required documentation has been sent. If everything is in order, the Registrar will award the credential. When the credential arrives, it will be mailed to the student’s home.
If you need your credential reprinted, you will need to fill out a Credential Reprint Request Form. The fee is $20 for each credential you want reprinted. Your credential request will be part of the credential order for that semester and it will take 8-10 weeks after the end of the semester for your credential(s) to be sent to you. Unless otherwise specified, your credential reprints will be mailed to the address we have for you in PeopleSoft.
