Please read and check all the boxes before filling out the rest of the drop request.
That my financial aid may be affected by this action and I will contact Financial Aid. If dropping all my classes, I have met with and have permission from Financial Aid to drop.
That it is my responsibility to pay any balance on my bill not covered by Financial Aid.
That my academic progress may be affected by this action. I will contact my advisor
If it is after mid-term, I have permission from my instructor to drop the class.
If I am dropping all my classes, I have permission from someone on the Student Affairs Leadership Team to drop.
This form will be processed as of the day the last permission email is received in the Registrar’s Office. (It must be received in the Registrar’s Office by 4 PM on the dates listed on the academic calendar.)
I must submit all requested information below for this drop request to be processed.
Attention: Email permission from any of the above listed groups is acceptable if the email is sent by Financial Aid, the instructor, and/or the Student Affairs Leadership Team member to email@example.com.
