If it is after mid-term, I have permission from my instructor to drop the class.
After mid-term, this form will be processed as of the day the last permission email is received in the Registrar’s Office. (It must be received in the Registrar’s Office by 4 PM on the dates listed on the academic calendar.)
I must submit all requested information below for this drop request to be processed.
